It's basically the best example I've ever found of how to write an AU well. (Spoilers and mental health stuff abound, though, so do heed the tags!)

I'll start. Home with the Faeries is a Lord of the Rings fanfic that takes the girl in Middle-earth premise that mars so many suefics and plays it straight as uncooked spaghetti and does it well. Really well. I love Maddie, the OC, and the author has a fantastic grasp of the canons' personalities. And I dare you to find me a Boromir/OC fanfic that's done so well. That's right, it ships a canon and an OC and does it well. Don't look at me like that. Just read it. You'll see what I mean.
Now go forth and show us what's great about fandom!
Stealing Harry is, from what I can tell, an interesting AU where Lucius Malfoy went to Azkaban for killing Peter Pettigrew instead of Sirues. This results in Sirius and Remus running a bookshop to keep an eye on Harry from a distance, and eventually rescuing him from the Durslay's when he's eight.
Overall, it's well written and it doesn't look like anyone's particularly OOOC.

A terrific story that takes an in-depth look at the affect Discord had on the characters and what comes after. It's more of a psychological look at the effects Discording would have on a pony... and the aftermath is not always pleasant. But the story is well-written and engaging, and incorporates previous My Little Pony series in an interesting way.

The story is a delightful skewering of the "Villain of the Week" trope with plenty of twists and turns. And it does incorporate canon... up to a point, anyway.

this is a fic about Mary Sues... all the while taking a deconstructive, and even deep, look into the concept. I can't recommend this one enough for people who like Mary Sue deconstructions.

A gloriously cathartic read as the Winchester brothers (and Castiel) poke holes into the vampires' smug, self-aggrandizing ways.

It's incomplete as of now, but it's not on hiatus and the author has definite plans to finish it. It's just really, really beautiful, and also very accurate when it comes to skating (the footnotes are so helpful).
Next is a Supernatural RPF fic, so most of you probably won't be interested in reading it. For those who do like RPF, though, it's Jared/Jensen in a spy AU. And it's actually well-done and really adorable, I loved it.

Sorry they're both on AO3. I know that's not what people tend to use around here, but I vastly prefer it to FF.net, so that's where I read fanfic.
As stated above, this is an original work, and even though you repeatedly said “fan works”, I wanted to share it with you. There’s yandere in there, so I would hesitate to call it realistic, but it’s pretty amusing/entertaining. It’s incomplete and on a hiatus, though.

It's called Antithesis, it is an AU of Harry Potter, but I believe it's well written.
Yes, it is a WBWL story, but it works well. It's an emotional story that makes you cry, rage and cheer on the characters. It also depicts the canon characters fairly well.
