Welcome aboard! I've just started watching Star Trek TOS (I'm on episode 10 of season one, so no spoilers!) and of course everyone's always happy to have another Sherlockian. I've been spending my morning looking at the fan theories on how he survived... so how do you think he survived?
I haven't done much theorizing myself, but I do have a way that Moriarty may have survived...
I'm not certain how one could fake eating a gun, but you have to admit, the camera angles were suspicious.
Everybody seems to think that the bouncy ball had something to do with preventing John from getting the pulse on a (presumably live and genuine) Sherlock-on-the-sidewalk, but I'm somewhat suspicious: it seems too obvious for it to be anything other than a red herring. Still, maybe it had other uses we weren't aware of...
My idea is that he fired a blank bullet into his mouth and had a small tube with blood attach to the back of his head through his hair. There would have been a seal that ruptured when his head hit the ground, and the pressure of his back on a sack of blood in his coat would keep it flowing. Bear in mind that we don't see his body get taken away.
I just rewatched the scene, and he's still shooting something directly into the roof of his mouth at approximately shot-out-of-a-pistol meters per second. Just because it's not a real round doesn't mean it wouldn't be blasted directly into his skull.
Is it possible he had a titanium plate on the roof of his mouth?
The titanium plate would actually help redirect some of the blast down his gullet.

I also think I saw a tweet from one of the writers saying something along the lines of "Yes, Moriarty shot himself point-blank in the head. No, we can't show that much gore on TV."
