As well as the locking, which Ansela's explained, lj also has adult content filters. So say you'd written a graphic and gory non-con fic. You'd mark it with the highest category of filter. Then, if anyone was logged into lj and tried to view your fic, if their stated birthdate makes them under 18, they'll be denied access. People who aren't logged in have to click a button confirming their age. It's not going to stop anyone from reading what you've written, but that extra level of attempt, on lj's part, to make sure underage people go where they're going willingly and with warnings, covers them slightly more legally (I presume) and means they're not so fussed about really graphic stuff.

On the other hand, don't ever specifically say a character is under 18 in an unlocked post if they're making with the sexytimes. There was an awful lot of "the intarwebz are exploding!" over that a while back.
